Accepted Insurance

Ascentist ENT participates with many insurance providers, including Medicare and most commercial plans. Even so, within those commercial plans are certain networks/policies that consider our office to be out-of-network. While we try our best to verify this when scheduling your appointment, it is ultimately your responsibility to confirm that our physicians are in network with your particular policy. Please contact your insurance provider at the number listed on your card. Reference our practice’s Tax Identification Number (481238955) for more precise results. See the chart below for known out-of-network insurance plans.

Referrals- If your insurance requires that you have a referral to be seen in our office, it is your responsibility to arrange this prior to your appointment. If a valid referral is not provided, you will be required to pay in full or be asked to reschedule your appointment. We realize this is an inconvenience, but if you are seen without a referral your insurance will deny payment for your care and you will be financially responsible.
